The hospital review,<\/a> carried out by Dr Andrew Fraser and Dr Brian Montgomery, was commissioned to find out if the management and execution of the project \u201cadversely impacted on the risk of healthcare-associated infection\u201d at the hospital.<\/p>\n

It was commissioned following the deaths of two people<\/a> at the facility who had contracted a fungal infection found in pigeon excrement. While the review concluded the design, build, commissioning and maintenance was not linked to the avoidable deaths, it found that defects in the pipework of water and air-ventilation systems may have presented an increased risk to infection.<\/p>\n

The hospital had multiple installation defects in its pipework and ventilation systems<\/strong>. According to reports to the QEUH Independent Review, installation defects included the wrong type of steel being used for pipes, pipework and fittings corrosion, as well as use of pipework from manufacturers that was not approved. Some of the pipework was also found to have been left open during installation work, making it vulnerable to contamination and, in some cases, left susceptible to small creatures entering the pipes. The specification for the build made it clear that pipework should be protected by caps to stop dirt, creatures and frost from entering.<\/p>\n

High levels of dust<\/strong> were also reported during the construction phase, but there was no physical or recorded evidence found of this.<\/p>\n

The report said: \u201cThese installation issues collectively represent potential threats to the integrity of the water system and would pose an increased risk to infection unless there are active steps to address these issues.\u201d<\/p>\n

Air-ventilation issues<\/strong> in the children\u2019s haemato-oncology ward, and unsealed ceilings<\/strong> in isolated treatment rooms were highlighted as issues, but the report said the contractor \u201cwas working to the correct design\u201d. Further reports on the design and construction of this ward were withheld from the review for legal reasons, but it noted witness statements that identified problems such as \u201cabnormal\u201d ductwork connections and the use of a thermal wheel, which increases energy efficiency but risks cross-contamination across air ducts and through ceilings such as those used in the ward.<\/p>\n

The report stated the design-and-build contractor \u201cdeclined to be interviewed formally<\/strong>\u201d, which prevented the collection of evidence<\/strong> regarding quality control from the contractor and its supply chain. There was also a lack of documentation around the standards of finish in sealed areas such as behind walls and above ceilings. As a result of this, one of the recommendations was the greater use of digital technologies to log and share project documentation.<\/p>\n

The review concluded that all contractors and subcontractors need to understand the implications of changes made to standards of healthcare projects before undertaking work.<\/p>\n

A Multiplex spokesman said: \u201cThe independent review has established no sound evidential basis for asserting that avoidable deaths have resulted from failures in the design, build, commissioning or maintenance of the QEUH and RHC.<\/p>\n
