At every stage, from design through occupation into facility management, wide gaps existed in knowing whether the right products had been specified, delivered, and actually used. This lack of assurance, she argued, undermined accountability and put residents\u2019 lives at risk.&lt;\/p&gt;  Seven years on, the Building Safety Act has attempted to close these gaps by embedding the \u201cgolden thread\u201d of information.
