{"id":534484,"date":"2025-12-10T11:55:17","date_gmt":"2025-12-10T11:55:17","guid":{"rendered":"https:\/\/www.constructionnews.co.uk\/?p=534484"},"modified":"2025-12-10T12:42:34","modified_gmt":"2025-12-10T12:42:34","slug":"hse-still-investigating-crane-deaths-incident-more-than-eight-years-on","status":"publish","type":"post","link":"https:\/\/www.constructionnews.co.uk\/health-and-safety\/hse-still-investigating-crane-deaths-incident-more-than-eight-years-on-10-12-2025\/","title":{"rendered":"HSE still investigating crane deaths incident, more than eight years on"},"content":{"rendered":"<p>The Health and Safety Executive (HSE) is still investigating a crane collapse that killed three men, almost eight-and-a-half years after the tragedy, <em>Construction News<\/em> can reveal.<\/p>\n<p>Rhys Barker, 18, from Castleford, West Yorkshire, David Newall, 36, from Bradford, West Yorkshire, and David Webb, 43, from Northampton, died following <a href=\"https:\/\/www.constructionnews.co.uk\/sections\/news\/crewe-crane-collapse-victims-were-falcon-tower-crane-workers-26-06-2017\/\">the crane collapse at a Seddon site on Dunwoody Way, Crewe, Cheshire, in June 2017<\/a>.<\/p>\n<p>Barker and Newall were crane erectors, while Webb (pictured) worked as an electrician. Seddon was building an 84-bed care home at the time.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-367903 alignleft\" src=\"https:\/\/cdn.ca.emap.com\/wp-content\/uploads\/sites\/8\/2017\/08\/David-Webb_Falcon_Cheshire-Police-300x200.jpg\" alt=\"David Webb\" width=\"300\" height=\"200\" \/><\/p>\n<p>An initial six-year investigation by the HSE concluded in 2023 and <a href=\"https:\/\/www.constructionnews.co.uk\/health-and-safety\/charges-over-three-deaths-in-crewe-crane-collapse-03-10-2023\/\">charges were brought against Norfolk-headquartered Falcon Tower Crane Services<\/a> under Section 2(1) and Section 3(1) of the Health and Safety at Work etc Act 1974.<\/p>\n<p>Falcon went on trial last year but the HSE\u2019s evidence was deemed not to meet the legal test for crown prosecutors and the judge directed a jury to find the company not guilty.<\/p>\n<p>According to barristers at <a href=\"https:\/\/www.hendersonchambers.co.uk\/2025\/05\/15\/alerter-hse-ordered-to-pay-the-entirety-of-the-defences-costs-for-a-prosecution-which-should-not-have-taken-place-and-was-starkly-improper-r-v-falcon-tower\/\">Henderson Chambers<\/a>, the watchdog was ordered to pay more than \u00a3587,000 to Falcon for its defence costs after the HSE was found to have brought the prosecution in a way that included an unnecessary or improper act or omission.<\/p>\n<p><em>CN<\/em> has established that the watchdog nevertheless considers its investigation to still be ongoing with potential action arising from it incomplete.<\/p>\n<p>An HSE spokesperson confirmed: \u201cAn investigation into one line of enquiry remains ongoing.\u201d<\/p>\n<p>He said the watchdog could not comment on any other aspect of the investigation.<\/p>\n<p><a href=\"https:\/\/www.constructionnews.co.uk\/legal\/unsafe-practices-led-to-crane-collapse-deaths-07-10-2025\/\">Earlier this year, an inquest found<\/a> that staff errors and unsafe practices led to the tragedy.<\/p>\n<p>Sitting at Cheshire Coroner\u2019s Court, senior coroner Jacqueline Devonish recorded verdicts of accidental death \u201ccontributed to by a series of errors in producing the method statement and the absence of a partial ballast\u201d.<\/p>\n<p>She said there had been a number of mistakes made in preparing documents outlining how the crane should be set up, which led to \u201cfalse confidence\u201d in the paperwork produced that had not later been spotted.<\/p>\n<p>This was compounded by the supervisor not following the build plan and sequence for the work correctly.<\/p>\n<p>\u201cThe change in process caused the jib to slew in the opposite direction to that planned, therefore endangering the local community when the crane fell onto two residential homes outside the site,\u201d the coroner said.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The Health and Safety Executive (HSE) is still investigating a crane collapse that killed three men, almost eight-and-a-half years after the tragedy, Construction News can reveal.
